How to Obtain Services

Self-Referral

With the Patient Portal, you can easily refer yourself for care by completing a self-referral form online. This feature allows you to initiate care without needing to contact the office directly. Simply fill out the form, and our team will follow up with you to schedule an appointment.

 

Ability to Cancel or Reschedule an Appointment

Life can be unpredictable, Please call us 24 hours in advance to modify or cancel your appointment. 

Admission Forms

When you first register, you'll be able to complete necessary admission forms online, including:

  • Consent Forms: These forms ensure that you understand and agree to the medical services provided.
  • Rights & Privacy Forms: These forms inform you about your rights as a patient and how your private information will be handled.
  • Demographic Forms: To help us better serve you, we request basic demographic information (such as contact details, insurance information, and emergency contacts).
  • Clarity Consent Forms: These forms ensure that you are fully informed about the services and treatments being provided, with a clear understanding of your responsibilities.

Sliding Fee Information

We understand that healthcare costs can be a concern. To assist with this, we offer a sliding fee scale based on your income. You can view the sliding fee schedule and submit your income documentation directly through the portal. Our Service are offered to individuals and families in the community who may not have access to mental health services due to costs. You do not have to be a Metropolitan Ministries client to receive services.
